Learning Objectives:

Describe the organizational structures of the Pennsylvania Pharmacists Care Network (PPCN) and Highmark Wholecare and how their partnership has evolved to meet the needs of patients.
Discuss the services implemented by community pharmacies through the community pharmacy enhanced service network and health plan partnership.
Describe the preliminary outcomes of community pharmacy enhanced patient care services in partnership with a health plan.

Session Description: This session will be a panel discussion including the Pennsylvania Pharmacists Care Network (PPCN), a statewide clinically integrated network of over 200 community pharmacies, and one of their valued payer partners, Highmark Wholecare (formerly Gateway Health) Medicaid Managed Care Organization of Pennsylvania. Together, PPCN and Highmark Wholecare will discuss the evolution of their partnership since 2017, the structure of their innovative programs and contracts, the pharmacy-provided services for eligible health plan members, the ongoing analysis of impact of these services on quality and cost outcomes, and any preliminary patient outcomes that can be shared – both qualitative and quantitative.
