Multidisciplinary Medication Management in an Ambulatory Dual-Eligible Patient Population

Session Description: This session will describe an on-going longitudinal multidisciplinary interoperable technology enabled medication management service for a PACE program. A comprehensive medication management software platform will be reviewed which imports disparate sources of medication and medical data and harmonizes multiple medication claims value sets, enabling determination of best possible reconciled medication regimen. This reconciled regimen is the basis for longitudinal comprehensive medication review. Automated risk rule algorithms provide medication risk assessment, including PQA quality measures. Medication review summaries are available for patients and are provided to PACE and ambulatory PCP providers via a unique electronic device platform, which incorporates individual patient frailty scores, and seamlessly promotes clinical documentation of pharmacists' recommendations response, and adjustments to the patient's medication regimen.
